Baldwin Park (hereinafter referred to as Agency) and pursuant to the auth ority und
Government Code section 21173, after review of medical and other evideric
relevant thereto, 1 hereby determine that Jorge Huerta, a local safety member of th
California Public Employees' Retirement System (CalPERS) employed by t
Agency, is incapacitated within the meaning of the California Public Employee
Retirement Law for performance of his duties in the position of Police Officer.
I hereby certify under penalty of perjury that this determination was made on the basis of
competent medical opinion and was not used as a substitute for the disciplinary process in
9ccordance with Government Code section 21156(a)(2).
